I have finally compiled the complete list.

P.S.  I'm sorry to post such a long message, but plenty of people expressed
      an interest in this info and I don't know how to make the data available
      on BOVIK.  If someone explains how this is done, I could store the actual
      (much longer) messages I received.

P.S.  If you sent me mail, but do not wish for it to be made publicly 
      accessible since I did not mention that I would do so, then just
      send me a short note to that affect.
