Lots of faculty and staff at CMU belong to Penn Group, myself being one.
You might get a feeling for how satisfied we are by calling the Benefits
Office in Warner Hall.  I have been satisfied with their services, which
include eye exams, psychiatric visits, ... many more than I have been able
to take advantage of, probably, and visits included in their premium.  My
costs have been prescription medications, eyeglasses, etc., but no extra
costs for tests, etc. that they performed.  The Benefits people know what
they provide better than  I do.  Let me know if there is something specific
you need to know about.
