Harry,

I just gathered some information on the current passport application
process.

  Allegheny County Passport Office
  Courthouse Room 222
  355-5770

Call this number, and you will get an answering machine with a very
detailed message about the application process. You have to appear in
person at the Passport Office with a copy of your birth certificate
(a certified copy with the seal of your state of birth) or appropriate
naturalization papers; (2) 2"x2" passport photos; your Social Security
number; proof of identification (a valid driver's license, photo work ID,
etc.); a check for $42. 

If you want to renew a passport, you can call the above number and leave
your name and address and they will send you the appropriate forms; the
renewal process can be done by mail.
